Manhole and Vault Safety

A manhole or a utility vault does not look dangerous. There is no height to fall from, no blade spinning, no obvious warning. There is just a round lid in the pavement and a dark space below it, the kind of opening crews step in and out of every day without much thought. That ordinariness is exactly the trap. The air inside an enclosed underground space can be deadly, and it gives you almost no warning before it drops you. Oxygen gets pushed out by other gases. Hydrogen sulfide and carbon monoxide collect with no reliable smell. A space that was fine an hour ago can be fatal now. People do not get hurt in manholes because they are reckless. They get hurt because the danger is invisible and the entry felt routine.
Here is the hard truth that drives this entire course: most people who die in confined spaces die during a quick, unplanned entry, and a large share of them are coworkers who climbed in to help. The hazard does not announce itself, so a worker drops down for a quick task with no permit and no testing, goes still, and someone rushes in after them and goes still too. That pattern repeats across the country, year after year. The good news is that it is preventable, completely, by following a few non-negotiable rules: test the air before and during entry, ventilate, post a trained attendant outside, set up non-entry rescue before anyone goes down, and never, ever enter on a hunch.
